ASSESSMENT Case Managers perform ongoing assessments that include activities that focus on strengths and needs identification. Activities include assessment of the individual to determine any need for medical, educational, social and other services. Specific assessment activities include: individual history, identifying the needs and preferences of the individual, and completing related documentation. It also includes gathering information from family members, medical providers, educators, employers and others who are important to the individual being assessed. PERSON CENTERED PLANNING Case Managers provide Person Centered Planning (PCP). Service planning builds on the information collected through the assessment and ensures the individual or family is at the center of and in control of the planning process. The individual and family determine needs, control choices and are respected for the individual’s dignity and worth. The person centered plan includes building systems of support to promote stability and a healthy lifestyle in a natural community setting.
REFERRAL & LINKAGE Case Managers provide referral and linkage to community resources that help the individual connect to medical, social, and educational services. Case Management services include locating, and coordinating natural and community supports. Case Managers make referral and arrangements for medical treatment, physical or psychological evaluation and treatment.
Monitoring Case managers provide monitoring and follow-up that includes activities and contacts necessary to ensure that the PCP is successfully implemented and adequately addressing the needs of the individual.
