Recovery
Services are strength-based and focused on promoting recovery. CST is designed to assist an individual that is at risk of homelessness, deterioration of mental health status, impairment due to co-occurring diagnostic issues, and designed to assist an individual in acquiring mental health/substance abuse recovery skills necessary to successfully address his/her educational, vocational, and housing needs. CST is an intensive community rehabilitation service that provides treatment and restorative interventions to assist individuals to gain access to necessary services and develop optimal community living skills.
TEAM APPROACH
Team members inform individuals about benefits, community resources, and services; assist them in accessing benefits and services; arrange for the individual to receive benefits and services; and monitor the provision of services. The team consults with identified providers, include their input into the service planning process, inform identified stakeholders, and monitor the status of the individual in relationship to the person-centered goals. Staff of Community Innovations assumes the role of advocate for all of its consumers receiving CST. Individuals will experience decreased crisis episodes, and increased community tenure, time working and/or in school or with social contacts, and personal satisfaction and independence.
The service activities consist of a variety of interventions including:
· Intervention to address barriers for independent functioning in the community
· One-on-one interventions in the community to develop interpersonal and community coping skills
· Assistance and support for the individuals in crisis situations
· Service coordination
· Psycho-education and support for individuals and their families
· Development of interpersonal, community coping and independent living skills
· Development of symptom monitoring and management skills
· Monitoring medication and self medication
